摘要： Background: The effect of worsening renal function and baseline chronic kidney disease (CKD) on outcomes in patients with chronic coronary syndrome in the setting of optimal medical therapy remains unknown. Methods and Results: The REAL-CAD (Randomized Evaluation of Aggressive or Moderate Lipid Lowering Therapy With Pitavastatin in Coronary Artery Disease) study is a prospective, multicenter, randomized trial of high-dose (pitavastatin 4 mg/day) or low-dose (pitavastatin 1 mg/day) statin therapy in 12 118 patients with chronic coronary syndrome. The primary end point was a composite of cardiovascular death, nonfatal myocardial infarction, stroke, or unstable angina requiring hospitalization (major adverse cardiac and cerebral events [MACCE]). CKD was defined as an estimated glomerular filtration rate [eGFR] <60 mL/min per 1.73 m(2). WRF was defined as a decrease in eGFR >= 20% in the initial year;borderline renal function was an annual decrease of 0%

摘要： Purpose: To compare the effect of post-and-core material type and production technique on the fracture resistance of teeth. Materials and Methods: Sixty human maxillary central incisors were used for the study. Root canal treatments were performed, and the post cavities were created. The teeth were divided into six groups (n = 10) according to post material and production method: prefabricated glass-fiber post (PF zirconia post (PZ titanium post (PT and CAD-CAM produced glass-fiber post (CF zirconia post (CZ titanium post (CT). All post-and-core restorations were luted with resin cement (RelyX U200). Static loading was applied on the luted metal copings using a universal testing machine to measure fracture resistance. The effect of material and production method on fracture resistance was evaluated with two-way ANOVA test. Tukey HSD test was used in post hoc analyses. Significance was evaluated at p < 0.05 level. Results: In the CAD-CAM post-and-cores, the fracture resistance of zirconia was significantly higher (809 N) than titanium (569 N) and glass-fiber (342 N). In the prefabricated posts, the fracture resistance of zirconia was significantly higher than glass-fiber (445 N while there was no significant difference between zirconia (605 N) and titanium (538 N) materials. Conclusion: Zirconia posts demonstrated the highest and CAD-CAM glass-fiber posts demonstrated the lowest fracture resistance compared to other types of post materials. Additional studies are needed to confirm if CAD-CAM glass-fiber posts may be considered an effective alternative treatment.

摘要： Extensive research to date has focused on the remediation of heavy metal-contaminated soils by various soil washing techniques. Many of the most effective techniques, however, rely on inorganic acids and/or chelating agents that can result in the deterioration of soil quality and other secondary environmental impacts. In this paper, a new environmentally friendly method for heavy metal-contaminated soil remediation, based on the invasive Opuntia ficus-indica (L.) Mill. is evaluated. The method involves the use of extracts prepared from O. ficus indica cladodes, harvested at phases I (Night) and III (Day) of the plant's photosynthetic cycle, as a soil washing agent. These green extracts were evaluated for the removal of copper (Cu) from three different soils including kaolinite, montmorillonite and a field-natural soil sample. Results indicated that undiluted night phase extracts yielded the greatest Cu removal rates in all three soils with an optimal washing duration of 24 h. Copper removal rates ranged from 56% to 85% and were significantly impacted by the intrinsic properties of the soils, the leachate dosage, the plant harvest phase, the number of washing cycles and the soil washing duration. The chemical characterization of the leachates indicated that the organic acids present in the O. ficus indica extracts as well as other functional groups are key players in the removal process. The obtained results demonstrated the potential of this plant extract for a sustainable abatement of heavy metal-contaminated soil.

摘要： BackgroundThe continuous development in digital prosthodontics allowed the customization of attachments and retentive inserts which offers an easy and cheap solution for regular maintenance of locator overdentures during daily practice. The present study compared the change in retention values of the fully digitally manufactured custom-made locator attachment retentive insert with the ready-made ones after insertion, removal, and masticatory cycles.MethodsA complete denture was constructed over a mandibular edentulous epoxy model. Two implants were inserted into the model between the laterals and canines following the prosthetically driven implant protocol. Locator retentive attachment inserts were digitally designed using free-form modeling software and milled from PEEK (Poly Ether Ether Ketone). After the pick-up of the ready- and custom-made retentive inserts, an insertion, removal, and masticatory cycles test simulating 1 year of patient usage was performed. The change in retention values was recorded at baseline, 6 months, and after one year of simulated clinical use. An independent sample t-test was used to compare the data between the two studied groups.ResultsThere were statistically significant differences in retention values between the custom-made and ready-made locator inserts at baseline and after 6 months. (p = 0.001*). On the other hand, there was no significant difference in retention after 1 year of simulated use. (p = 0.083, NS)ConclusionsThe custom-made milled locator retentive attachment insert can be used as an alternative to the ready-made one due to their comparable retention values after 1 year of simulated use.

摘要： Purpose of ReviewInflammatory bowel disease (IBD) can cause significant psychological, physical, and economic burdens on patients and healthcare systems. Studies show over one-fifth of patients will seek nontraditional methods of treatment for managing their symptoms. Understanding the benefits - and potential harms - of these therapies is important to provide holistic and evidence-based care to our IBD patients.Recent FindingsIn this review, we present several studied herbal therapies for the management of both Crohn's disease and ulcerative colitis. These include cannabinoids, Tripterygium wilfordii, Chios mastic gum, Boswellia serrata, Indigo Naturalis, curcumin, resveratrol, and Zingiber officinale. While these herbal remedies have been shown to have anti-inflammatory effects and positive outcomes in IBD patients, larger scale studies are lacking and the use may be limited by bioavailability, lack of standardization of formulations, and adverse reactions.SummaryIn reviewing the literature, we discuss the current data available including benefits, adverse reactions, and considerations for use surrounding several of the more common herbal remedies used for IBD.

摘要： This study presents a novel method for creating customized brain slice matrices using Computer-Aided Design (CAD) and 3D printing technology. Brain Slice Matrices are essential jigs for the reproducible preparation of brain tissue sections in neuroscience research. Our approach leverages the advantages of 3D printing, including design flexibility, cost-effectiveness, and rapid prototyping, to produce custom-made brain matrices based on specific morphometric measurements. The developed protocol is user-friendly and incorporates features such as embossed identifying markers and support structures for challenging thin regions, thereby enhancing its practical utility. Our method demonstrates rapid and cost-effective fabrication of custom brain matrices, significantly reducing both material expenses and production time compared to traditional manufacturing techniques and ready-made products. This work contributes to the growing application of 3D printing in biomedical research, offering a valuable tool for neuroscientists requiring precise and consistent brain tissue sectioning.

摘要： Background Anatomically formed healing abutments were suggested in literature to address many of the issues associated with immediate posterior implant insertion such as large extraction sockets that are extremely hard to seal without reflecting the mucoperiosteal flap, extraction sockets anatomy that are not suitable for regular healing abutment placement, and potentially high occlusal stresses when planning a temporary implant supported prothesis to improve the conditioning of supra implant tissue architecture and the emergence profile of the implant supported restorations. Purpose To clinically evaluate the peri-implant soft tissue profile of single posterior implant retained restorations and to assess patient related outcomes of the implant restorations that were conditioned immediately by CAD-CAM socket sealing abutments (SSA) versus those conditioned by Titanium (Ti) standard healing abutments (SHA). Methods Twenty participants received twenty-two single maxillary immediate implants after flapless minimally invasive tooth extraction and 3D guided implant placement in the posterior area (premolar and molar) and allocated randomly into two groups (n = 11 the intervention group: patients received PEEK SSA and the control group: the patients received Ti SHA. Modified Pink Esthetic Score (PES) was evaluated at 3 observation periods: Baseline T0 (immediate after implant supported crown insertion 6 months T1 and 1 year of clinical performance T2. Patient satisfaction was assessed one week and one year after crown insertion using visual analogue scale (VAS). Results At base line, after six as well as 12 months, SSA group showed statistically significant higher total modified PES scores than SHA group (P-value < 0.001). 摘要： Precise detection of crop disease at the early stage is a crucial task, which will reduce the spreading of disease by taking preventive measures. The main goal of this research is to propose a hybrid classification system for detecting crop disease by utilising Modified Deep Joint (MDJ) segmentation. The detection of crop diseases involves five stages. They are data acquisition, pre-processing, segmentation, feature extraction and disease detection. In the initial stage, image data of diverse crops is gathered in the data acquisition phase. According to the work, we are considering Apple and corn crops with benchmark datasets. The input image is subjected to pre-processing by utilising the median filtering process. Subsequently, the pre-processed image under goes a segmentation process, where Modified Deep Joint segmentation is proposed in this work. From the segmented image, features like shape, colour, texture-based features and Improved Median Binary Pattern (IMBP)-based features are extracted. Finally, the extracted features are given to the hybrid classification system for identifying the crop diseases. The hybrid classification model includes Bidirectional Long Short-Term Memory (Bi-LSTM) and Deep Belief Network (DBN) classifiers. The outcome of both the classifiers is the score, which is subjected to an improved score level fusion model, which determines the final detection results. Finally, the performance of the proposed hybrid model is evaluated over existing methods for various metrics. At a training data of 90%, the proposed scheme attained an accuracy of 0.965, while conventional methods achieved less accuracy rates.

摘要： Nitric oxide (NO)-stimulated cyclic guanosine monophosphate (cGMP) is a key regulator of cardiovascular health, as NO-cGMP signalling is impaired in diseases like pulmonary hypertension, heart failure and chronic kidney disease. The development of NO-independent sGC stimulators and activators provide a novel therapeutic option to restore altered NO signalling. sGC stimulators have been already approved for the treatment of pulmonary arterial hypertension (PAH), chronic thromboembolic pulmonary hypertension (CTEPH), and chronic heart failure (HFrEF), while sGC activators are currently in phase-2 clinical trials for CKD.

摘要： Coronary computed tomography angiography (CTA) analysis can help in the planning of percutaneous coronary intervention (PCI). Fractional flow reserve derived from coronary CTA (FFR_CT), coronary CTA-derived regional myocardial mass, and FFR_CT virtual PCI planner can facilitate decisions concerning sheath and guide catheter selection, stent lengths on the basis of predicted post-PCI FFR_CT, optimal fluoroscopic angles, evaluation of provisional vs 2-stent bifurcation PCI techniques, and assessment of the magnitude of jeopardized myocardial mass in cases with side branch compromise. This case series illustrates the emerging opportunities for coronary CTA-based planning of bifurcation PCI.
